Obtain the following:
1) a copy of your birth certificate (see resource link)
2) DS-11 passport application form – or complete the form on line (see resource link) DO NOT SIGN IT UNTIL THE PASSPORT AGENT TELLS YOU TO
If you were born outside f the U.S., you will need one of the following instead of the birth certificate:
Consular Report of Birth Abroad or Certification of Birth
Certificate of Citizenship
You will also need one of the following for identification:
Valid Driver’s License
Current Government ID (city, state or federal)
OR Current Military ID (military and dependents)
For the ID being used, you will need to make a copy of both sides of it on 8-1/2″ x 11″ standard white paper with no other markings. This is regular printer size paper. It cannot be larger or smaller. You can only use ne side of the paper so if you can’t get both sides on the same paper, you can use two sheets.
You will also need two passport photos that meet the following requirements:
2 x 2 inches in size
Taken within the past 6 months, showing current appearance
Full face, front view with a plain white or off-white background
Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
Taken in normal street attire
Itmis easiest to go to a photoshop or drugstore or anywhere else that advertises passport photos. hey know the rules and most will guarantee acceptance of the photos.
You can take your application and fee to a passport agency and they will make sure everything is in order and they will submit it for you. Some of the post offices are passport agents and many city halls are – even the small city hall I went to to get my passport
You may have to go and purchase a money order for the fee. The city hall I went to only accepted money orders – no cash, checks, or credit cards. See the resource reference for the passport acceptance facility. There are twenty of them within 10 miles for me.
